Glossary
This glossary describes the fields and tabs of the two main screens of the module: Accident/Incident Notifications and Accident/Incident Investigations.
«Accident/Incident Notifications» Screen
Section titled “«Accident/Incident Notifications» Screen”Fields
Section titled “Fields”- Type of accident: indicates whether the record is Accident with leave, Accident without leave or Incident.
- Date of accident/incident: date on which it occurred.
- Day of the week: automatically filled in when the date is entered.
- Time of accident/incident: time at which it occurred.
- Working time: time of the shift when it occurred. It is mandatory from Smart OSH (not requested on anonymous page, App or Viewer app).
- Affected person: free text to identify the affected employee when the record comes from anonymous page, App or Viewer app. The application searches and automatically fills in the Affected employee from this data.
- Affected employee (requires selecting a scope): selectable affected employee. The available options depend on the scope indicated in the notification.
- Description of the accident/incident location: free text with details of the location.
- Description of the work of the injured person: free text with details of the work performed.
- Description of the accident/incident: free text with what happened. If the record comes from external channels, it also allows noting the names and surnames of witnesses.
- Proposed causes: free text with the proposed causes.
- Proposed measures: free text with the proposed measures.
- Tags: specific filter to classify the record.
- Status: phase of the notification. By default, Pending review when created; Closed when the investigation is created. Other statuses are assigned manually.
- Closing date: closing date of the notification. Automatically filled in when the investigation is created.
- Accident/incident investigation code: automatically filled in when the investigation is created through the notification.
- Scope: scope to which the notification belongs.
- Witnesses: identifies the people who witnessed the accident/incident.
- Responses: entry of responses to the notifier and consultation of previous ones.
- Attachments: documents added to the notification. Their link is included in the management email to the responsible person.
- Responsible persons: automatically filled in according to the Management Responsibilities defined in Organisation.
- Inform to: automatically filled in according to the Information Responsibilities defined in Organisation.
- Notifier data: automatically filled in when registering the record (provided the user data is available).
- SmartStatus: alerts automatically generated for this notification.
List screen filters
Section titled “List screen filters”- Inactive status: search filter View active that shows all records with relationships associated with their scope as Inactive.

Section titled “«Accident/Incident Investigations» Screen”General fields
Section titled “General fields”- Type of accident: Accident with leave, Accident without leave or Incident.
- Investigated by: person responsible for carrying out the investigation.
- Affected employee (requires selecting a scope): affected employee. Options depend on the scope. If the investigation comes from a notification, this field is locked.
- Date of accident/incident.
- Day of the week: automatically filled in when the date is entered.
- Time of accident/incident and Working time.
- Description of the accident/incident: details of what happened.
- Proposed causes and Proposed measures.
- Tags.
- Status: phase of the investigation. By default, In progress when created; other statuses are assigned manually.
- Accident/incident notification code: automatically filled in when the investigation is created from a notification.
Specific checks
Section titled “Specific checks”- Relapse: indicates that the record comes from a relapse. Only activatable for records of type Accident with leave. When checked, it enables the field Original accident investigation.
- Original accident investigation: original accident investigation related to the relapse. Only active if Relapse is checked.
- Occupational disease: indicates that the record is related to an occupational disease.
- Other occupational health damages: indicates if there are other occupational damages.
- Suspected case: indicates that the record corresponds to a suspected case.
- Completed / Communicated: indicates that the investigation is completed and/or communicated.
- P.E.P. No.: occupational disease report number.
Actions
Section titled “Actions”- Report: action that shows the report only on screen at the moment. It is not saved in the programme.
