How to create a questionnaire template
Questionnaire templates are the basis on which all questionnaires in the module are generated. In the template, you define the general data, the questions, and the associated automatisms.
Procedure
Section titled “Procedure”- Access the Questionnaires module → Configuration → Questionnaire templates and click New.
- Complete the general data. Fields marked with an asterisk (*) are mandatory.
- Expand the information through the configuration tabs (see below).
Configuration checks
Section titled “Configuration checks”Among the general data, you will find several checks that allow you to adjust the behaviour of the questionnaire:
Requires anonymity
Section titled “Requires anonymity”If checked, when submitting the questionnaire, the data of the person who completed it is not collected in the Completed by field.
Show score
Section titled “Show score”If checked, the Questionnaire report will display the score for each question and the total score obtained.
Employee observation
Section titled “Employee observation”If checked, when completing the questionnaire, a box below each question will appear where the employee can leave observations.
External companies as scope
Section titled “External companies as scope”If checked, you will be able to select an external company as the scope of the questionnaire. When the employee accesses the questionnaire, the option External company will appear in the scope field.
Training
Section titled “Training”If checked, the questionnaire will be used exclusively for the Training module and will not be available in App | Questionnaires.
Do not show in the App
Section titled “Do not show in the App”If checked, the questionnaire will not be available for use in App | Questionnaires.
Configuration tabs
Section titled “Configuration tabs”Once the template is created, you have several tabs to finish configuring it:
- Organisational structure: indicates for which level of the structure you want this questionnaire to be visible. If you do not specify any, the questionnaire will be visible to any user. If you specify a position, you must also include the company, site, and/or section that apply.
- Questions: configure the questionnaire questions, choosing the type and marking the correct answers. See How to configure questions.
- Help text: enter a text that will be shown to the employee to help them complete the questionnaire correctly.
- Automatic report sending: configure the automatic sending of the completed questionnaire report, either to the author or to any other email address.
- Scores: specify the general scores of the questionnaire (score for correct answer, incorrect, unanswered, and minimum score).
- Periodicity: generate a sending periodicity for this questionnaire.
