How to configure questions and scores

The questions are the core of any questionnaire template. On this page, you will see how to create them, configure the different available question types, assign automatic actions to them, and parameterise the scores.

  1. Access the questionnaire template and open the Questions tab.
  2. Click New.
  3. Enter the statement and choose the question type: Yes/No, Numeric, Date, Time, Currency, Single selection, Multiple selection, Signature, Title, etc.
  4. Go to the Configuration tab to define the possible answers.
  5. If needed, add explanatory text in the Contextual help section.
  6. If desired, parameterise the individual score of the question in the Score section.
  1. When selecting this type, the possible answers automatically appear in the configuration tab: Yes, No, Not applicable, and Don’t know / No answer.
  2. To modify an answer, access it via the edit icon. From the screen that opens you can:
    • Change the text of the answer and its description.
    • Mark if it is the correct answer.
    • Mark if the answer is not applicable.
    • Assign an individual score.

Configure Single or Multiple selection questions

Section titled “Configure Single or Multiple selection questions”
  1. Select the Single selection or Multiple selection type.
  2. Click New to create the first answer.
  3. In the window that opens, specify:
    • The answer text in the Value field.
    • An optional description.
    • Whether it is a correct answer or not applicable.
    • The associated score.

Configure Date, Time, Currency or Numeric type questions

In these question types, answers are defined by expressions that delimit intervals.

  1. Select the corresponding question type (Date, Time, Currency or Numeric).
  2. Click New to create the first answer.
  3. In the window that opens, define the answer using an expression of the type answer to the question (numeric) – Equal to – X.
  4. Fill in the Expression defining the interval field, add a description, and mark if it is correct or not applicable. Assign a score if appropriate.

Questions in questionnaires can trigger automatic actions depending on the given answer (for example, generating a risk factor, a measure, a task, or a non-conformity).

  1. Open the Questions tab and enter the corresponding question.
  2. Access the answer on which you want to apply the action.
  3. Open the Actions tab and click New.
  4. Indicate the action class you want to apply.

The parameterisation of questionnaire scores is done from the template itself.

  1. Enter the questionnaire template and open the Scores tab.
  2. Indicate the scores for each question type in the corresponding boxes.
